Fees for Support at Home packages

  • I am thinking about Home Care services but don’t know where to start.

    If you are over 65, you may be eligible for a government-funded Support at Home package.

    1. Register and Apply with My Aged Care (MAC)
    2. Phone Interview – Initial step to assess your eligibility
    3. Your Home Assessment – My Aged Care will follow-up the interview and assess your needs. See below for a list of documents you might want to have on hand*.
    4. Complete Your Means and Asset Assessment (Services Australia)
    5. Wait for your Support at Home program to start

    *For your Home Assessment, make sure you have the following at hand:

    • your Medicare card and one other form of ID – such as driver’s license, healthcare card, passport or DVA card.
    • a copy of any referrals from your doctor.
    • contact details for your GP or other health professionals.
    • information on any support you receive.

    Prepare for Support At Home. Its time to start thinking about selecting the right provider. While you wait for your Support at Home program to be funded, it’s also a good time to shop around and compare a selection of home care providers, their services, expertise, fees and charges.

  • I already have Home Care services and I want to switch to MiCare

    Some easy steps to switch your home care provider

    Need more from your home care provider. So – how can you make the switch?

    1.Shop around and choose a new provider
    Research and then call your short-listed new service providers and discuss what services they can offer, your needs and how they can accommodate the switch.

    MiCare can assist you through the entire journey of switching providers, including visiting your home and offering advice on any fees that may arise from making the switch. Call 1800 642 273 for more information.

    Whilst you shop around, we also suggest you read over your home care agreement with your existing provider. Take note of any exit fees and notice periods.

    2.Agree on a switch date
    Meet with your current service provider and come to a mutual agreement on a date you’ll finish receiving in-home support with them.

    My Aged Care gives you 56 days from the end date to enter into a new agreement with another service provider – so we suggest choosing your new provider within or before this time period.

    3. Notify My Aged Care

    Contact My Aged Care, to let them know you wish to transfer. They will reactivate your referral code (a unique number provided to you after your assessment, enabling you to access government-funded aged care services). Only give your code to a provider when you are ready to start services.

     4.Meet with your new provider
    Arrange to meet with your new provider. Take your reactivated referral code. In this meeting you can start to arrange your new home support plan and agree on a start date.

    You can’t overlap providers – so make sure your start date is immediately after your finish date with your old provider. This way will not experience any gaps in your in-home support.

     5. Take your funding with you

    To ensure you have your unspent funding come with you, you must give your old provider the details of your new provider within 56 days of ending services with them.

    You may be charged an exit fee – which is taken out of your unspent funds. Please ask your previous provider about this.

    6. Begin with your new provider
    Congratulations! You can now begin to receive in-home support with your new provider! Make sure to check your old provider transferred your unspent funds within the allotted 70 days.

You personal Home Care team.

Our Home Care team will be with you every step of the way. We will assign you your very own team of specialists, to provide personal care, nursing and support for every day living.

  • Before you start your services, we will come to your house, to understand your needs and talk you through the process
  • You are assigned a care partner. You can call them to discuss, manage and adapt your care package to make sure you always get the most out of your support at home program.
  • We will match you with a team of local carers, and try to make sure it’s the same people coming to your house on a regular basis. And if English isn’t your first language, we’ll make every effort to connect you with a carer who speaks your language.

 

Expert care for specialised health needs.

Many people with higher-level Support at Home funding have clinical needs, such as diabetes, continence issues, heart and lung diseases, dementia, joint pain, a history of falls, and generalised weakness or frailty. We arrange clinical assessment and monitoring by a Registered Nurse, as our experience has been that this greatly enhances wellbeing. With the involvement of a Registered Nurse, we are able to provide valuable information and advice to those with high care needs,  and can potentially access additional funding for people with some conditions.

Social Support Group

I attend the MiCare Social Support Group to connect with people, share a nice meal, and enjoy the company—I would truly miss it if I couldn’t attend.
